There are not many public health issues where views are as extremely polarized as those concerning vaccination policies. Ever since its Fast Track approval by the U.S. Food and Drug Administration in 2006, Merck's human papilloma virus vaccine Gardasil has been sparking controversy. Initially, the criticism has been focused at Merck, due to their overly aggressive marketing strategies and lobbying campaigns. According to a 2007 editorial in Nature Biotechnology, Surrounded by a chorus of disapproval, Merck cracked. As Nature Biotechnology went (...) to press, the company announced a cessation of all efforts to lobby for US state laws requiring compulsory vaccination. Subsequently, questions have been raised whether it was appropriate for vaccine manufacturers to partake in public health policies when their conflicts of interests were so obvious. Some of their advertising campaign slogans, such as cervical cancer kills x women per year and your daughter could become one less life affected by cervical cancer, seemed more designed to promote fear rather than evidence-based decision making about the potential benefits of the vaccine versus any risks. On Exceeding Determination and the Ideal of Reason: Immanuel Kant, William Desmond, and the Noumenological Principle examines the critical philosophy of Immanuel Kant, as it bears on theological principles. Focusing on the foundational ideas (of self, world, and God) that constitute Kant's metaphysical system, Shaw argues that these ideal projections of the rational structures of the thinking subject only conceal and obfuscate the more robust sense of the real that exists behind all phenomenal appearances. This book aims to critically (...) assess the theological content presented in the philosophy of Kant whilst reconstructing and developing some of Kant's more obscure points on the epistemological limitations of pure reason and the borders of knowledge itself. Shaw draws into dialogue with the writings of the contemporary philosopher William Desmond to demonstrate some of the problems of Kantian thought when it comes to the deeper mysteries of being. Desmondian philosophy proves to be a powerful influence over much of this work, as Shaw advances upon the delicate nexus where philosophy and theology convene. As a bold addition to this work, Shaw lays the groundwork for a new discourse in theology and the philosophy of religion: "Noumenology". Noumenology, which is Shaw's response to the deficiencies in methods concerning many of the modern approaches to phenomenology, is a call for the recalibration of the starting point for eidetic inquiry through the proposal of a non-discursive approach to the sensible, emotional, and intellectual - or perhaps, the all together spiritual. To accomplish this, Shaw reaches back to Kant for a heterodox approach to the language and content surrounding the noumenonal to set forth his theological interpretation of the "Noumenological Principle". This book provides a broad survey of many of the major areas in neuroplasticity research by leading investigators in the field. The topics considered range across all levels of nervous system organization from the molecular to behavioral levels for species ranging from _C. elegans_ to humans. In addition, the effects of development and neuropathological events are discussed. A final summary chapter synthesizes the data gathered in this volume in order to provide the basis for a general theory of neuroplasticity.

Deliberative and participatory approaches to democracy seek to directly include citizens in decision-making and agenda-setting processes. These methods date back to the very foundations of democracy in Athens, where regular citizens shared the burden of governance and deliberated every major issue. However, thinkers at the time rightly believed that these methods could not function beyond the scale of the city-state, or polis. Representative democracy as an innovation improved on the scalability of collective decision making, but in doing so, sacrificed the (...) extent to which regular citizens could participate in deliberation. Modern technology, including advances in computational power, machine learning algorithms, and data visualization techniques, presents a unique opportunity to scale out deliberative processes. Here we describe Polis, an open source web application capable of collecting and synthesizing feedback from people in a scalable and distributed fashion. Polis has shown itself capable of building shared understanding, disincentivizing counterproductive behavior, and cultivating points of consensus. It has done this in the context of journalistic and academic research, and directly as part of decision-making bodies at local and national levels, directly affecting legislation. These results demonstrate that deliberative processes can be scaled up beyond the constraints of in-person gatherings and small groups. (shrink)

Qualitative and quantitative research on the impact of medical and health humanities teaching in baccalaureate education is sparse. This paper reviews recent studies of the impact of medical and health humanities coursework in pre-health professions education and describes a pilot study of baccalaureate students who completed semester-long medical humanities courses in the Division of Medical Humanities & Bioethics at the University of Rochester. The study format was an email survey. All participants were current or former baccalaureate students who had taken (...) one or more courses in literature and narrative in medicine, bioethics, history of medicine, and/or visual arts and healthcare during the past four years. The survey gathered numerical data in several areas: demographic information, career plans, self-reported influence of coursework on educational and career plans, and self-reported influence of coursework on intellectual skills and abilities. It also gathered narrative commentary that elaborated on students’ responses to the numerically-based questions. Notable findings from preliminary analysis of the data include higher scores of self-reported impact of the coursework on specific habits of mind and on preparedness for intended career rather than on gaining admission to future educational programs. Discussion of the results focuses on several potential future directions this type of study might take, including multi-center, longitudinal, and sequential approaches. (shrink)

In this article we summarise some previously described proposals for ethical governance of autonomous vehicles, critique them, and offer an alternative solution. Rather than programming cars to react to crash situations in the same way as humans, having humans program pre-set responses for a wide range of different potential scenarios, or applying particular ethical theories, we suggest that decisions should be made jointly between humans and cars. Given that humans lack the requisite processing capacity, and computers lack the necessary ethical (...) capacity, the medical paradigm of advance care planning can be retooled for this new context. Advance car-crash planning provides a way to combine humans’ ethical preferences with the advanced data processing capacities of computers to enable shared decision making in collision situations. Reorienting the Political examines the reception of two controversial German philosophers, Carl Schmitt and Leo Strauss, in the Chinese-speaking world. This volume explores the powerful resonance of both thinkers in Chinese political thought from a cross-cultural and interdisciplinary perspective.

Background Our objective was to evaluate whether the description of a machine learning (ML) app or brain imaging technology to predict the onset of schizophrenia or alcohol use disorder (AUD) influences healthcare professionals’ judgments of stigma, empathy, and compassion. Methods We randomized healthcare professionals (N = 310) to one vignette about a person whose clinician seeks to predict schizophrenia or an AUD, using a ML app, brain imaging, or a psychosocial assessment. Participants used scales to measure their judgments of stigma, (...) empathy, and compassion. Results Participants randomized to the ML vignette endorsed less anger and more fear relative to the psychosocial vignette, and the brain imaging vignette elicited higher pity ratings. The brain imaging and ML vignettes evoked lower personal responsibility judgments compared to the psychosocial vignette. Physicians and nurses reported less empathy than clinical psychologists. Conclusions The use of predictive technologies may reinforce essentialist views about mental health and substance use that may increase specific aspects of stigma and reduce others. (shrink)

If ever a prize is to be given for the most cantankerous figure in the history of science and medicine, Almroth Wright will surely be on the short list. With the exception of himself, Wright was a man who did not tolerate fools lightly. Even the author of this biography, who tries very hard to see Wright's point of view, becomes exasperated with him at times. More commonly known today as Sir Colenso Ridgeon in George Bernard Shaw's The Doctor's (...) Dilemma, Wright was born of stern, fanatical Irish Protestant parents in 1861. Extremely gifted, Wright graduated first in modern languages at Trinity College, Dublin. After switching to and qualifying in medicine, a gadfly postgraduate career followed. His first permanent job was as pathologist at the army's medical headquarters at Netley near Southampton, where Wright developed the agent for which his name remains inscribed on the bacteriologists' roll of honor: prophylactic typhoid vaccine. Its passage into common use, however, was far from easy. Statisticians, clinicians, and military authorities lined up against him. Wright's belief in the vaccine's value was vindicated in World War I. The controversy over its efficacy has been analyzed by J. Rosser Matthews in his Quantification and the Quest for Medical Certainty . Dunnill does not refer to this, but, there again, scarcely any modern secondary source is recognized in this book. This is not that sort of a biography, which is not to decry its great merits. Dunnill is a pathologist who sticks very close to the primary sources, including large amounts of hitherto‐unpublished manuscript material. His approach is very evaluative of what Wright got scientifically right and wrong by today's criteria, but the judgments are made in a subdued and relatively unobtrusive way. This is a great virtue in dealing with any aspect of the volatile Wright. His views on woman's place in the world and her claim for the right to vote would drive any intolerant feminist historian to apoplexy. Dunnill, however, takes great pains to situate Wright's views in context.A man like Wright was never going to be happy at Netley since the military hierarchy weighed on him, crushing his individualism. Only the dictatorial control of an autonomous empire would suit him, and this is what he eventually got. Wright became the head of the pathology department at St. Mary's Hospital, London, and there he was effectively responsible to nobody. This was possible because he turned his laboratory into a vaccine‐producing factory and used the drug company Parke Davies to market his products. These included not only prophylactic agents but now‐discredited ones for the treatment of the then‐common bacterial diseases such as pneumonia. The Great War saw Wright in France struggling to introduce another innovation that was finally accepted. “Listerian antisepsis” was a hallowed phrase among surgeons, and the more infected war wounds became the more intense became their antiseptic applications—sadly, to little effect. Wright campaigned for simple saline washing of wounds, a practice eventually adopted with much better results. After the war, separated from his wife, at odds with the Medical Research Council, and estranged from some of his colleagues , Wright turned to the consolations of philosophy—unfortunately writing rather than just reading it. His otiose productions solving the riddle of the universe now lie unread. This conventional but highly readable biography offers no new insights into the science of the period, but it does merit attention as a study of the relations of personality and empire building. This paper considers the tension between timelessness and timeboundedness in legal interpretation, examining parallels between sacred texts and secular law. It is argued that familiar dualities such as those between statute and judge-made law, law and equity, written and spoken discourse, dictionary meaning versus intended or contextual meaning, can be examined using this timeless/timebounded framework. Two landmark English cases, DPP v Shaw (1961) and R v R (1991) are analyzed as illustrating contrasting aspects of the socio-legal politics of “reasoning (...) backwards”. The related temporal distinction between ex ante and ex post points of view is examined both within legal theory and as a key issue for linguistic and semiotic systems. The argument is made that this distinction is the key to a wide range of methodological and theoretical problems in relating linguistics and semiotics to law. (shrink)

Although populism is an ideologically fluid political vehicle, it is not one that is intrinsically anti-business. Indeed, different varieties of populist parties may encourage business activity for utilitarian ends, but with their own ideas on what businesses should be doing. This reality implies that initiatives not related to national greatness or priorities as defined by the populist leadership may be viewed as redundant. Key among such initiatives would be corporate social responsibility (CSR). In a populist environment, it is possible that (...) firms may divert resources away from broad-based CSR under pressure from populist governments. This article explores the relationship between populist governance and CSR with an econometric examination of over a thousand firms in 13 countries under both pro- and anti-business populist governments at varying times from 2012 to 2020. Using dynamic panel data methods, we find strong evidence that firms substitute away significantly from CSR under populism. This effect grows significantly larger under anti-business populists. Professional engineers are under increasing pressure to practice in an environmentally sensitive way. To prepare engineers for this new reality, changes in engineering education are needed. For example, engineering hydrology has traditionally been taught with an emphasis on the interpretation of numerical data bout rainfall and runoff in watersheds. However, to do environmentally sensitive hydrology work, it is necessary to also understand the life forms that share the watershed. In 1997, a project was undertaken in the Department of Civil Engineering (...) at the University of Toronto to enhance an introductory hydrology course by adding information about how hydrological phenomena affect fish. Through carefully structured assignments and exam questions, an assessment was made of the effectiveness of the enhancements in increasing students’ awareness of the life context of hydrology. The project has resulted in a commitment to increased environmental information in the hydrology curriculum, and the implementation of an assessment process for all students, to monitor changes in their environmental knowledge and attitudes. It has also raised questions about ultimate objectives in engineering education. (shrink)

Marleen Eijkholt presents a new argument in healthcare ethics, the false hope harms (FHH) argument. In brief, false hope promotes a host of individual harms (e.g., financial, physical, and psychological harms) and system‐level harms (e.g., distrust of medical practitioners, increased complexity of care and the associated costs), all of which provide reason for healthcare providers to stop promoting false hope in medicine. The goal of this paper is to show that the FHH argument is unsuccessful.

The present study examines section five of Poroi and Xenophon’s proposal to restore the reputation of Athens. After outlining his plan for ‘justly’ supplying the dēmos with sufficient sustenance in Poroi 1-4, section 5 addresses the desire to regain hegemony after Athens had lost the Social War. Xenophon does not adopt an anti-imperialist stance; instead he seeks to re-align imperial aspirations with Athenian ideals and earlier paradigms for securing hegemony. Xenophon’s ideas in Poroi are contextualized with consideration for his ‘Socratic’ (...) distinction between tyranny and kingship, as well as his wider advice for ruling well. It is shown that his proposals for securing the consent of the allies reiterates ideas that Xenophon outlines across his corpus, especially Hiero and Cyropaedia. In Poroi Xenophon therefore applies his political thought in an attempt to re-direct Athenian ambitions away from policies that prompted charges of being a ‘tyrant polis’ and towards ‘legitimate rule’. Theory and practice, reason and revelation, ancients and moderns are the key themes running through the eighteen studies of the literature of political philosophy in Reason's Inquisition. Alfarabi is a pivotal figure, but the range is wide, from Plato and Thucydides to Shakespeare and Hobbes, extending to such contemporary figures as Leo Strauss and Eric Voegelin.

Discussions of monotheism often consider its bigotry toward other gods as a source of conflict, or emphasize its universality as a source of peaceful tolerance. Both approaches, however, ignore the combined danger and liberation in monotheism's 'intolerance.' In this volume, Christopher Haw reframes this important argument. He demonstrates the value of rejecting paradigms of inclusivity in favor of an agonistic pluralism and intolerance of absolutism. Haw proposes a model that retains liberal, pluralistic principles while acknowledging their limitations, and he (...) relates them to theologies latent in political ideas. His volume offers a nuanced, evolutionary, and historical understanding of the biblical tradition's emergence and its political consequences with respect to violence. It suggests how we can mediate impasses between liberal and conservative views in culture wars; between liberal inclusivity and conservative decisionism; and, on the religious front, between apologetics for exclusive monotheism and critiques of its intolerance. The year 2019 witnessed the 20th Jubileum of the Växjö conference series on quantum foundations and probability in physics. This has been the longest running series of conferences on the subject in history. Many old and new friendships were forged at Linnaeus University and the beautiful surrounding lakes of Småland, where once yearly everyone gathers to renew the debate and report their latest progress. 2019 also represents the Porcelain Anniversary—18 years—of the point of view on quantum theory known as QBism. (...) In this regard, the 2001 meeting in the series was pivotal in more ways than one. Not only did it instigate Fuchs’s break [1] from the earlier (Jaynes style) “objective Bayesianism” and neo-Copenhagen thinking of Caves et al. [2], but it was the first meeting at an international scale to indicate that the quantum information revolution might have something genuine to contribute to quantum foundational thought. That meeting set the pace far beyond Växjö by the participation of a number of luminaries of quantum information: The late Asher Peres, Daniel Greenberger, David Mermin, Herbert Bernstein, Carlton Caves, Rüdiger Schack, Richard Jozsa, Benjamin Schumacher, and John Smolin. One could almost hear a battle cry echo from the woods, “Quantum foundations from quantum information or die!” The foundations debate had turned so stale by the time, it would surely die without an influx of new thinking. In a familiar passage from Plato's Euthyphro, Socrates points to a contrast between "matters of difference that cause hatred and anger," and matters where agreement is reached by seemingly rational means. Where a dispute concerns number, size or weight, we arrive at a decision by counting or measuring. But there are matters of disagreement where such convergence is not to be expected: "the just and the unjust, the beautiful and the ugly, the good and the bad" notorious among them. Socrates's (...) point, in drawing what a modern reader might take to be a contrast between facts and values, is not so easily understood. The Euthyphro passage elicits a concern-call it a concern about "objectivity"-which is often addressed in a characteristically narrow way. How we approach a given concern depends on how we articulate that concern, and this is what I propose to consider here. (shrink)

Religion has become a vital resource for attempts to rethink the meaning of the political. This article rehearses the efforts of two recent figures, René Girard and Giorgio Agamben, to transform the political by renewing its connection to religion. Both thinkers struggle to escape politics as defined by Carl Schmitt's friend/enemy distinction. Girard and Agamben do clash ideologically, but their inquiries into sacrifice and messianism take similar courses. Regarding origins, Girard argues for the sacrificial crisis as the common parent to (...) religion and politics. Conversely, for Agamben, the Roman figure of homo sacer distinguishes politics from religion. With respect to the future, Girard's messianism installs Christian belief as the only way to move beyond violence. By contrast, Agamben steers Pauline messianism toward the efforts to displace sovereignty and reopen the political. I conclude that Agamben breaks with Schmitt while Girard reinscribes his politics at a higher level. The purpose of this qualitative case study is to understand and critique the gendered political processes in the search for an athletic director following a merger between men’s and women’s intercollegiate athletic departments in a U.S. university. Semi-structured interviews were used to ask 55 athletic department stakeholders their perceptions of the search process and associated politics. Findings indicated gendered political activities occurred along gender-affiliated departmental lines. Political strategies contributed to gendered processes favoring certain masculinities and male candidates in the search (...) for an athletic director. While gender equity was an important consideration in the search process, because of the controversial nature of the merger and the politics expressed, the belief that hiring a man was essential to the merger’s success was widely accepted by many stakeholder groups. The findings are positioned in the critical management and sport literature. This research contributes to our understanding of the complexity of gender relations and provides insight into the gendered political processes that inform leadership searches. (shrink)

In ‘Dilemma for Appeals to the Moral Significance of Birth’, we argued that a dilemma is faced by those who believe that birth is the event at which infanticide is ruled out. Those who reject the moral permissibility of infanticide by appeal to the moral significance of birth must either accept the moral permissibility of a late-term abortion for a non-therapeutic reason or not. If they accept it, they need to account for the strong intuition that her decision is wrong (...) as well as deny the underlying normative principle that killing a viable fetus requires good reason, and not wanting to care for the child when the child could be easily placed for adoption is not a good enough reason to abort. If they reject the moral permissibility of the late-term abortion, they need to explain why her decision is wrong. Doing so, however, will undermine their own project of denying infanticide by appeal to birth. Walter Veit argues that the dilemma relies too much on intuition and does not live up to biological continuity. We explain why his criticisms are unconvincing. (shrink)

The book as a symbol of totality and logocentric order has become a familiar motif in histories of medieval culture. In recent years numerous studies have examined in detail not only how this “idea of the book” and its dependent metaphors rendered all experience “legible,” but also how the tasks of ensuring a correct “reading” devolved upon exegesis and commentary.

In the coming decades, highly treated wastewater, known as reclaimed water, is slated to be a major element of municipal water supplies. In particular, planners propose supplementing drinking water with reclaimed water as a sustainable solution to the growing challenge of urban water scarcity. Public opposition is currently considered the primary barrier to implementing successful potable water reuse projects; nonetheless, public responses to reclaimed water are not well understood. Based on a survey of over 250 residents of Tucson, Arizona, this (...) article assesses the relationship between trust in the professional institutions responsible for municipal water development and willingness to drink reclaimed water. Results demonstrate that public acceptance of potable reuse is contingent on trust in the authorities who influence design of sociotechnical systems for water supply and reuse—including water and wastewater utilities, regulators, consultants, academics, and elected local officials. Findings emphasize the highly interdependent social and political factors that inform personal decisions to support or reject potable reuse. The authors suggest that achieving greater acceptance of potable reuse will require bringing local and regional water policy in line with public values, as well as finding ways to incorporate these values into the planning process. There is a growing consensus among philosophers that hope is a moral virtue: the virtuously hopeful person experiences the right amount of hope for the right things. This moralization of hope presents us with a puzzle. The historical consensus is that hope is a passion and hope is a theological virtue, not a moral virtue. Thomas Aquinas, the philosopher who wrote most extensively on hope, offers an explanation for why hope is not a moral virtue. Patients and physicians do not know the cost of medical procedures. Opaque medical billing thus contributes to exorbitant, rising medical costs, burdening the healthcare system and individuals. After criticizing two proposed solutions to the problem of opaque medical billing, I argue that the Centers for Medicare and Medicaid Services should pursue a rule requiring that patients be informed by the physician of a reasonable out-of-pocket expense estimate for non-urgent procedures prior to services rendered.

Recent discussions surrounding the conceptualising of disability has resulted in a stalemate between British sociologists and philosophers. The stagnation of theorizing that has occurred threatens not only academic pursuits and the advancement of theoretical interpretations within the Disability Studies community, but also how we educate and advocate politically, legally, and socially. More pointedly, many activists and theorists in the UK appear to believe the British social model is the only effective means of understanding and advocating on behalf of people with (...) disabilities. This model, largely reliant upon materialist research traditions, contends that disability is a form of social oppression and hence, is a phenomenon that should be conceptualised in social terms. Individual properties such as impairments are disregarded as they are viewed to be unimportant in the analysis of the social causes of disability. Concurrently, many bioethicists and philosophers have embraced what Tom Shakespeare has classified as an ‘Interactional Approach’ to disability—that “the experience of a disabled person results from the relationship between factors intrinsic to the individual, and the extrinsic factors arising from the wider context in which she finds herself”. I intend to demonstrate that the benefits of the British social model are now outweighed by its burdens. I suggest, as Jerome Bickenbach has, that while it may be somewhat churlish to critique the social model in light of its political success, taken literally, it implies that people with disabilities require no additional health resources by virtue of their impairments. Despite the eloquent arguments that have preceded me by interactional theorists, none have been accepted as evidence of fallacious reasoning by British social model theorists. This article is an attempt to clarify why it is that the types of arguments British social model theorists have been offering are misguided. I suggest that the British social model, unlike an interactional approach, is unable to provide a realistic account of the experience of disability, and subsequently, unable to be properly utilized to ensure justice for people with disabilities.
